Overview:
Discharge planning is a process that starts at the point of admission and follows beyond discharge through the continuum of care. This workshop will cover a wide range of topics related to discharge planning, including how to engage with other members of the interdisciplinary care team when planning for the patient’s movement across the continuum. Learn how to ensure that your discharge planning processes address the complexities of the new healthcare environment as well as the Centers for Medicare & Medicaid Services (CMS) regulatory requirements. This webinar series is designed for registered nurses and social work case managers. This series is useful, both for those who are new to the process and those who want to upgrade their role in discharge planning.

Registration fee is $600 for all 4 webinars, or $225 each individually. This is a per hospital registration fee with no limit on the number of staff participating. When registering, indicate “All 4 webinars – Discharge Planning” or for which individual webinars you are registering – example: “Part 1 & 3 – Discharge Planning”.
Part One: 8/28/25 from 9:00 – 11:00 a.m.
– Understand discharge planning as a process, not an outcome.
– Discuss the new CMS changes related to transitional and discharge planning and how they can impact your practice.
– Identify best practice strategies for transitioning patients across the continuum of care.
Part Two: 9/4/25 from 9:00 – 11:00 a.m.
– Review examples of case management assessments that meet regulatory requirements from CMS.
– Understand the new requirements from CMS related to social drivers of health and how to assess patients for these drivers.
– Apply the regulatory requirements related to discharge planning.
Part Three: 9/11/25 from 9:00 – 11:00 a.m.
– Identify strategies that can be used to reduce your hospital’s readmission rate.
– Review definitions and examples of the continuum of care and how it applies to your discharge planning process.
– Discuss the levels of care, their differences, and the patient requirements for acceptance into one of these levels.
Part Four: 9/18/25 from 9:00 – 11:00 a.m.
– Identify specialty roles in case management that support the discharge and transitional planning processes.
– Review strategies for working with post-acute providers.
– Discuss discharge planning issues.
Speaker:
Bev Cunningham has been a partner and consultant with Case Management Concepts, LLC, since its inception. Previously, Bev served as the vice president, resource management at Medical City Dallas Hospital where she was responsible for case management, health information management, patient access, solid organ transplant, and transplant financial services. Bev has been involved in the development of case management for over 25 years, and her areas of expertise include the role of case management in the revenue cycle, compliance for case management departments, the role of the physician advisor, and effective denials and appeal management. She has achieved ACM certification by the American Case Management Association.
Toni G. Cesta is a founding partner of Case Management Concepts, LLC, a consulting company that assists institutions in designing, implementing, and evaluating case management departments and models, educational programs, and onsite support for leadership and staff. She has been active in the research and development of case management for over 25 years. Her research has included two funded studies measuring the effects of a case management model on congestive heart failure and fractured hip patient populations, with measures of patient satisfaction, quality of life, and short- and long-term clinical perceptions and outcomes.
